Standards for preadmission information and house rules are as follows:

  1. The provider shall furnish each prospective resident, or the resident's conservator, guardian, relative, or other person responsible for placement, the following information prior to entering the home:
  1. Restrictions and limitations on the use of alcohol and tobacco.
  2. Restrictions and limitations on the use of the telephone.
  3. Sample menus of meals served.
  4. Procedure concerning the use and management of resident funds.
  5. Procedure used for billing and collecting the charge for board, room, and care.
  6. A policy addressing whether or not refunds will be made for pre-paid board, room, and/or care.
  7. Policies concerning the furnishing of non-emergency resident transportation by the provider.
  8. A statement of other relevant house rules with which the resident will be expected to comply such as the privacy of others, visitation by family members or friends, etc.
  9. Accurate and complete information regarding the extent and nature of the care available from the provider.
  10. Procedure for the distribution of mail and for sending mail.
  1. House rules must be reviewed by the provider and dated as current for license renewal.
